Health Care Program for Children in Foster Care (Health Passport)

Public Health nurses are located in county child welfare service agencies and probation departments to give expertise on medical, dental, mental and developmental needs of children and youth in foster care. The nurses:

  • Advocate for health needs of the child
  • Provide medical case planning and coordination
  • Serve as a resource to facilitate referral to early intervention providers
  • Oversee the creation and updating of the Health and Education Passport
