Infectious Disease and Reporting Guidance

Please help our public health officials keep our community safe. This page will help you report all reportable infectious disease cases to the right department. Healthcare providers in El Dorado County are required to report communicable diseases to the El Dorado County Public Health Division, Communicable Disease section, as specified in the Title 17, California Code of Regulations (CCR) §2500, §2593, §2641.5-2643.20, and §2800-2812 Reportable Diseases and Conditions

  • El Dorado County Public Health uses the system, California Reportable Disease Information Exchange (CalREDIE). The purpose of CalREDIE is to improve the efficiency of surveillance activities and the early detection of public health events through the collection of complete and timely surveillance information on a statewide basis. This allows for 24/7/365 reporting and receipt of notifiable conditions.
  •  Providers needing to report an Infectious Disease can do so by using the appropriate Confidential Morbidity Report via fax or electronically through the CalREDIE Provider Portal, see below:
  •  Confidential Morbidity Reports (CMR) forms are to be faxed to the following number (530) 295-2589. Use the following forms:
  •  CDPH 110A: Use this form for reporting all conditions except Tuberculosis and conditions reportable to DMV. Initial report required from provider for any reportable disease.
  • CDPH 110B: Only use this form for reporting Tuberculosis.
  • CDPH 110C: Use this form for reporting lapses of consciousness or control, Alzheimer's disease, or other conditions which may impair the ability to operate a motor vehicle safely (pursuant to H&S 103900).
  • CMR 110D: Only use this form for reporting COVID-19.

CalREDIE has created the Provider Portal (PP), a module provides secure access to healthcare providers to electronically submit Confidential Morbidity Reports (CMRs), required by California Code of Regulations (CCR), Title 17, Section 2500 directly to LHDs.
